The upside of being open
Celebrity transparency can be a powerful thing says Austin, TX plastic surgeonJohnny Franco, MD. “It’s opened the door to real conversations and empowered patients to set more realistic expectations,” he explains. “People are starting to realize that many of the flawless faces and sculpted bodies they admire aren’t just luck or genetics. They’re often the result of thoughtful enhancements over time.”
When public figures like Oprah acknowledge aesthetic choices, like using a GLP-1 medication, it sends a message says Dr. Franco. “It helps normalize the idea that beauty can be enhanced and that’s okay. This openness is redefining what’s possible and giving people permission to explore their own aesthetic goals without shame.”
Public transparency can also help patients feel less alone. “It allows patients to feel more comfortable exploring options for self-enhancement without shame or secrecy,” says New York plastic surgeon Mokhtar Asaadi, MD. “It helps dismantle outdated taboos.”
Southfield, MI plastic surgeon Mariam Awada, MD agrees. “When it’s honest, transparency can be empowering. It empowers everyday women to explore aesthetic choices without shame,” she says.
But is it all just spin?
So, what’s the problem? Not all celebrity confessions tell the full story say some experts. “There’s a difference between being transparent and being selectively transparent,” says Dr. Franco. “When people leave out key details or suggest their results are ‘natural’ when they’re not, it can set unrealistic expectations and create confusion.”
Even when celebrities seem to be upfront, Dr. Asaadi cautions there are still some smoke and mirrors being used on Instagram. “Social media is still highly curated. Face-tuning, filters and photo editing remain widespread and can distort perceptions of what’s real or achievable,” he says.
Some confessions, Dr. Awada notes, can end up leaving more questions than answers. “When celebrities claim it’s ‘just Botox’ but clearly have had more extensive work, it misleads women and fuels unrealistic expectations. It’s not empowering to glamorize results while hiding the truth. That creates confusion, not confidence.”
Influencing the exam room
Whether it’s a salmon sperm facial or a filler combo named after a celeb, one thing is clear, patients are paying attention. “There’s no question that celebrity influence and social media are driving beauty trends,” says Dr. Franco. “From specific procedures to overall aesthetic goals, patients are absolutely bringing in what they’ve seen on their favorite stars or influencers.”

It means patients are coming in more educated, curious and thoughtful about the decisions they’re making.”
That curiosity, while welcome, has its limits. “While it’s great that they come in informed, it’s essential that their care plan is based on their unique anatomy and goals,” says Dr. Asaadi. “Not a one-size-fits-all approach.”
Should we all admit to getting work done now?
As more influencers and celebrities embrace transparency, many patients wonder if they need to follow suit. “In certain circles, cosmetic treatments have become so normalized that there’s almost an expectation of openness,” says Dr. Asaadi. “At the same time, many patients still prefer discretion. Both perspectives are valid and patients should never feel pressured to explain or justify their choices.”
Some patients love sharing their journey. “It’s a celebration,” says Dr. Franco. “Whether it’s a mom who’s spent years putting everyone else first or a younger patient whose breast augmentation feels life-changing, these moments matter. They want to document it, track their progress and inspire others.”
But for others, staying private feels just as right. “Most patients prefer not to post or share on social media,” says Dr. Awada. “The few that do tend to be influencers or younger women. Women are expected to magically look flawless which can be exhausting. You don’t owe anyone an explanation for how you care for yourself.”
