QUESTION: I hold my Louisiana contractor’s license in one name, but I do business under a DBA (“doing business as”) name. Can I advertise, bid or contract under a DBA?
ANSWER: Anyone licensed by the Louisiana State Licensing Board for Contractors (“LSLBC”) must bid, contract, and perform work in his name as it appears on official records. LSLBC does not recognize DBAs, assumed names or fictitious names as legal business entities and it will not appear on official records, including the license.

ANSWER: Can a Medicaid decision be appealed?
Yes, one can ask for an appeal, which is also called a State Fair Hearing, if:
If all the services requested were denied
If part of the services requested were denied
If one offered different services from those requested
If the service provider did not ask for all of the services requested (Will need a doctor’s note explaining why these services are necessary when appeal submitted)
How does one appeal?
Can appeal by:
Filling out an appeal form online at: http://www.adminlaw.state.la.us/HH.htm or
Sending a written request for appeal to: Division of Administrative Law Health and Hospitals Section, P. O. Box 4189, Baton Rouge, LA 70821-4189, (fax) 225.219.9823 or
Calling: (225) 342-5800 or (225) 342-0443 (Phone appeals are allowed, but are not recommended)
****Choose only one way to file the appeal. One should not file multiple appeals for the same denied service.
Is there anything else that one can do to get services?
The provider who requested the services for can ask for a reconsideration with more information within 30 days of the denial.
A new decision will be received, and if services are denied again, one can appeal.
